Community Health Grant Eligibility

The ITA intends to fund projects that focus on one or more of the following areas:

  1. Racism & Discrimination: expand existing programs or test new and innovative approaches that support individuals within a specific community experiencing racism and/or discrimination.

  2. Social Isolation: expand existing programs or test new and innovative approaches that strengthen social connectedness & reduce social isolation among members of a specific community.

  3. Community Space & Support: expand existing programs or test new and innovative approaches for community-supportive spaces to improve social and emotional wellbeing.

This Invitation to Apply (ITA) is for 501(c)(3) organizations and 501(c)(3) fiscal-sponsored groups or programs. KPNW funding history does not play a factor in your ITA eligibility. However, organizations must predominately serve one of the groups mentioned above, and the organization’s leadership and staff must reflect the community/s it serves. If this is not possible due to workforce shortages, staff and leadership must have been trained on the specific cultures they serve, as well as on implicit bias and institutional and structural racism.

Additionally, applicants must be providing services in at least one of these counties of the KPNW service area of Oregon (Benton, Clackamas, Columbia, Hood River, Lane, Linn, Marion, Multnomah, Polk, Washington, Yamhill) and SW Washington (Clark, Cowlitz, Skamania, and Wahkiakum).

**IMPORTANT UPDATE — BUDGET TEMPLATE DOWNLOAD**

It has come to our attention that the project budget template linked in the Mosaic application includes an incorrect formula that limits the KP funding request to 33% of the total project cost. Please use & submit the template provided here. We apologize for the confusion.
